What Does the Science Say About D3 and Back-to-School? 5 Recent Studies Explained

Somewhere around the second week of school, the first cough moves down the hallway. Classroom settings bring exposure to a greater number of people in confined spaces, and germs try to find new hosts. Children’s immune systems go on high alert and begin responding to these new threats.

Vitamin D3 is a fat-soluble nutrient best known for supporting bone health, but it has a role in immunity as well. Here’s what they found.

1. Immune Cells Are Built to Use Vitamin D Directly

A comprehensive review published in Nutrients (2020) laid out the cellular mechanics behind vitamin D’s role in immunity. Monocytes, macrophages, dendritic cells, T cells, and B cells all carry vitamin D receptors, along with the enzyme needed to convert vitamin D into its active form right where they’re stationed. This local activation allows these cells to use vitamin D directly as they carry out their roles in the immune response — including differentiating into their mature, functional forms and coordinating with other immune cells.

This finding matters because it shows how vitamin D functions within the very cells responsible for detecting and responding to everyday exposures that come with crowded classrooms, shared supplies, and busy hallways. Vitamin D is processed and put to use locally, cell by cell, exactly where the immune system needs it most.

2. Vitamin D Status Is Linked to Respiratory Infection Outcomes

A large case-control study using UK Biobank data, published in the American Journal of Clinical Nutrition in early 2026, examined the relationship between vitamin D levels and hospital admissions for respiratory infections. Researchers found that people with severely low vitamin D levels (below 15 nmol/L; 6 ng/mL) had a 33% higher risk of hospital admission for a respiratory infection compared to those with sufficient levels (at or above 75 nmol/L; 30 ng/mL).

This kind of large-scale finding gives real-world weight to the cellular mechanisms described above. It shows that difference playing out in actual health outcomes across a large population. 

3. Vitamin D Supports the Gut’s Role in Immunity, Too

An international consensus statement published in Reviews in Endocrine and Metabolic Disorders in 2026 brought together a multidisciplinary panel of experts to review vitamin D’s role in gut health. Their review found that vitamin D supports a balanced gut microbiome, helps maintain the integrity of the intestinal barrier, and contributes to immune tolerance in the gut. All of these play a role in the body’s ability to fend off infection.

This adds an important dimension to the back-to-school picture. Cafeterias, shared water fountains, and close quarters spread more than respiratory illness. Plenty of the “stomach bugs” that circulate through schools each fall are gut-related, and gut immunity draws on many of the same vitamin D-dependent pathways.

4. Lower Vitamin D Levels Are Linked to More Frequent Ear, Nose, and Throat Infections in Kids

A 2025 study of 311 children and adolescents, published by researchers at a pediatric ENT clinic, examined the relationship between vitamin D levels and three common childhood infections: nasopharyngitis (the common cold), otitis media (ear infections), and rhinosinusitis (sinus infections). Nasopharyngitis affected 83% of the children in the study at least once over six months, and otitis media affected 53%.

Children with lower vitamin D levels had significantly more frequent episodes of both nasopharyngitis and otitis media. The relationship held up statistically across the full study group, with vitamin D levels and infection recurrence moving consistently in opposite directions: as vitamin D levels dropped, recurrence climbed. Because ear infections, colds, and sinus infections are among the illnesses that commonly interrupt the school year, these findings add another piece of evidence linking healthy vitamin D status with children’s immune resilience.

5. Early Vitamin D Levels Are Linked to Childhood Cognitive Outcomes

A longitudinal cohort study published in the American Journal of Clinical Nutrition in 2025, part of the Environmental Influences on Child Health Outcomes (ECHO) program, measured vitamin D (25(OH)D) levels in prenatal and cord blood and then assessed children’s cognitive performance at ages 7 to 12 using the NIH Toolbox Cognition Battery. The study found a measurable link between early vitamin D status and later cognitive outcomes, illustrating that vitamin D intake during pregnancy may have measurable long-term effects on a child’s cognitive development. While this research focuses on early development rather than school-age supplementation, it highlights the importance of maintaining healthy vitamin D status throughout life, beginning even before birth.
